The purpose of Supporting the Continuum of Care: Building Ryan White Program Grantee Capacity to Enroll Eligible Clients in ACA Supported Health Coverage is to build the capacity of RWHAP grantees and related providers to provide outreach to PLWH and to screen and enroll eligible PLWH in expanded health insurance during the annual open enrollment period beginning in October 2013. The announcement will fund one Cooperative Agreement to provide technical assistance to accomplish three main objectives: (1) assess RWHAP grantees’ technical assistance needs related to building capacity for outreach and enrollment of minority PLWH into expanded health insurance options; (2) work with HRSA, CDC, SAMSHA and CMS to assess best practices for minority client outreach and enrollment activities that would facilitate access to health care in the Medicaid expansion and Health Insurance Marketplaces (Exchanges); and (3) work with HHS agencies and national organization partners to develop strategies, tools, and trainings for RWHAP grantees to utilize for outreach and enrollment activities.

The geographic range of the proposal is nationwide and will focus on the racial and ethnic minority clients currently served by the RWHAP. The main activities for the three objectives are:
(1) To assess RWHAP grantees’ technical assistance needs related to building capacity for outreach and enrollment of minority PLWH into expanded health insurance options;
(2) To work with HRSA, CDC, SAMSHA and CMS to assess best practices for minority client outreach and enrollment activities that would facilitate access to health care in the Medicaid expansion and Health Insurance Marketplaces (Exchanges);
(3) Work with HHS agencies and national organization partners to develop strategies, tools, and trainings for RWHAP grantees to utilize for outreach and enrollment activities.

Grant Amount: The NIAID intends to commit $11 million total costs in FY 2014 to fund up to five awards.

The purpose of the Sexually Transmitted Infections Cooperative Research Centers (STI CRC) program is to facilitate multidisciplinary, synergistic collaborations that will increase our understanding and further our knowledge of co-infections of two or more specific STIs, as well as the interactions between and among different STI pathogens that may cause or enhance polymicrobial infections of the urogenital tract, e.g., bacterial vaginosis (BV), pelvic inflammatory disease (PID), intraamniotic infection (IAI) and non-gonococcal urethritis (NGU). This collaborative effort is expected to catalyze the discovery of new approaches to prevent, diagnose, and treat co-infections, polymicrobial infections and other clinical outcomes that affect reproductive health.

The objectives of this FOA are to support a group of STI CRCs to engage in a multidisciplinary, diverse set of STI research activities and to encourage new efforts towards understanding co-infections of known STIs, as well as infections that incorporate several pathogens (polymicrobial infections) and present as specific syndromes, for example pelvic inflammatory disease (PID). Basic, translational and/or clinical research is acceptable provided it addresses the purpose of this FOA.

It is anticipated that these efforts will provide insights into the biology of microbes, their interactions with each other, their role in pathogenesis, and their interactions with the host. To this end, the STI CRCs are encouraged to build upon current knowledge, i.e., to utilize data from existing microbiome studies to better define the role of normal flora in STIs; to develop and evaluate control and prevention strategies to reduce the public health impact of STI infections; and to share expertise in order to maintain a vigorous and productive research agenda. An important corollary objective is the training and mentoring of investigators new to the STI field through the establishment and management of pilot Developmental Research Projects.

Topics that would be considered responsive include the study of co-infections, e.g., organisms that present in association with each other or other STIs and are often treated syndromatically versus specifically, as well as polymicrobial infections of the urogenital tract, e.g., bacterial vaginosis (BV), pelvic inflammatory disease (PID), intraamniotic infection (IAI) and non-gonococcal urethritis (NGU).

Fund Title: Innovative Assays to Quantify the Latent HIV Reservoir (R21)

The purpose of this Funding Opportunity Announcement (FOA) is to support research on new or improved methods for quantifying latently infected cells in HIV-positive individuals on effective antiretroviral therapy. Quantifying latent HIV reservoirs is critical to evaluating strategies to cure HIV infection in vivo. The current gold-standard method involves a limiting dilution viral outgrowth assay that is slow, resource-intensive, and relatively imprecise. A molecular assay to accurately detect replication-competent provirus, or a viral outgrowth assay with improved efficiency, would facilitate proof-of-concept studies for curing HIV infection.

The goal of this FOA is to support research on the development of innovative high-throughput approaches for quantifying the very low levels of latently infected cells that persist in HIV-positive individuals on HAART. Ideally, a new assay is needed that specifically quantifies resting CD4 memory T cells that contain integrated, replication-competent provirus. The key challenges to be overcome are (1) the low frequency of these cells (only about 1 in 106 resting cells contains functional provirus), (2) the lack of cell surface markers distinguishing latently infected cells from uninfected cells, and (3) the abundance of defective HIV DNA (only about 1% of HIV DNA in resting CD4 T cells represents replication-competent provirus).

Examples of research that would fit within the scope of this FOA include, but are not limited to the following:

modifications to the current viral outgrowth assay to improve throughput, cost, precision, or accuracy;
technologies to adapt the viral outgrowth assay to a miniaturized, high-throughput platform; the
automation of steps in the viral outgrowth assay protocol; the use of advanced detection technologies to quantify latent HIV; and the development of a molecular assay that could serve as a surrogate for the viral outgrowth assay in quantifying replication-competent latent HIV reservoirs.

The P01 is a confederation of interrelated research projects, each capable of standing on its own scientific merit but complementing one another. The P01 application must include a minimum of two individual research projects that contribute to the program objective. Each individual research project should reflect a distinct, separate, scientifically meritorious research effort led by an independent investigator, the Project Leader. In addition, the individual projects should be clearly interrelated and synergistic so that the research ideas, efforts, and outcomes of the program as a whole will offer a distinct advantage over pursuing the individual projects separately.

In addition to individual research projects, applicants may propose one or more shared cores if needed for the proposed research.
